15 Program for Prevention and Management
for a Cardio-cerebrovascular Disease

For people with hypertension and diabetes, Korea CDC provides community-based programs which improve patients self-management skills to prevent their cardio-cerebrovascular disease that has a huge socioeconomic burden.

  • For people with hypertension and/or diabetes, Korea CDC provides community-based programs which improve patient’ self-management skills to prevent their cardio-cerebrovascular disease that has a huge socioeconomic burden.

Background of the Program

  • The socioeconomic burden of chronic diseases such as cancer and cardio-cerebrovascular disease is rapidly increasing. In particular, cardio-cerebrovascular disease is the second leading cause of death in Korea accounted for one-quarter of total death(2015, Statistics Korea).

Due to Korea’s aging population, the number of elderly patients with hypertension, cerebrovascular disease, osteoarthritis, and diabetes is also rapidly increasing, which leads to tremendous individual and socioeconomic burden.

Major chronic diseases require long term treatment. People with these conditions, however, can live a healthy life through their lifestyle changes. It is important to emphasize the importance of national- and community-level prevention and management programs for cardio-cerebrovascular disease.

Major Programs of the Program

  • Health Communication Campaign Program for 「The Prevention and Management for Cardiac and Cerebrovascular Diseases」
    • Every year, in the first week of September, Korea CDC with local governments and private institutions carries out campaigns for achieving community-level changes in risk behaviors through raising awareness of cardio-cerebrovascular disease and health practices.
  • Program to Support Hypertensive and/or Diabetic Patients for Continuous Health Care
    • Korea CDC provides 「Registration and Management Program for Hypertension/Diabetes」 in collaboration with community health center and private medical centers to community-dwelling patients with hypertension and/or diabetes for their continuous health care.

Registration and Management Program for Hypertension/Diabetes

1. Target population

Patients with hypertension or diabetes over 30 years old who are registered residents in the communities below

Seoul(Seongdong-gu), Gwangju(Gwangsan-gu), Ulsan(Jung-gu), Sejong(Sejong-si), Gyeonggi-do(Gwangmyeong-si, Namyangju-si, Ansan-si, Bucheon-si, Hanam-si), Gangwon-do(Donghae-si, Hongcheon-gun), Jeollabuk-do(Jinan-gun), Jeollanam-do(Mokpo-si, Yeosu-si, Jangseong-gun), Gyeongsangbuk-do(Gyeongju-si, Pohang-si), Gyeongsangnam-do(Sacheon-si), Jeju-do(Jeju-si)

2. Programs
  • Reduction in Client’s Medical Costs : For patents aged 65 years and older once a month

    1,500 won and 2,000 won for medical and medicine costs(for each condition), respectively (for both conditions, 4,000 won for medicine costs)

  • Reminder Service : Used to remind patients to attend for appointments on time for continuous treatment
  • Education and Counselling Service : For improvement of self-management skills including medication use, nutritional management, physical activity, etc.

  • Visit a medical center in your community and provide the consent form for the participation in the program

  • Which hospital or pharmacy can I register at?

    You can get your support at a hospital or pharmacy with the certification sticker on. Please check with your local public health center in advance.

  • What should I prepare when visiting a hospital or pharmacy?

    When visiting a hospital or pharmacy for the first time, make sure you bring your identification card or health insurance card which identifies you as a resident living in the business area. Please complete the consent form for 「Registration and Management Program for Hypertension and Diabetes」 before consulting a doctor.
