Purpose
To identify the current status of smoking harm in Korea and to calculate the socioeconomic burden caused by smoking.
Progress
Research on establishing a foundation for studying smoking harm and measuring the socioeconomic burden (2019–2021)
- Establishment of a research database on smoking harm, calculation of the number of deaths attributable to direct smoking and socioeconomic costs as of 2019.
Research on calculating deaths and socioeconomic burden that can be attributed to smoking (2022–2024)
- Calculation of the number of deaths attributable to direct smoking and socioeconomic costs for 2020–2022.

Process of Calculating Deaths Due to Smoking
- (Step 1) After selecting the total deaths and major diseases caused by smoking, calculate the risk for current and former smokers compared to non-smokers.
- (Step 2) Identify the smoking rate from approximately 20-30 years before the reference year for calculating the number of deaths, then calculate the population attributable fraction.
- (Step 3) Calculate the number of deaths due to smoking by multiplying the total number of deaths by the population attributable fraction.
Process of Calculating Socioeconomic Costs Due to Smoking
- (Step 1) After calculating the population attributable fraction by disease and gender, calculate the direct and indirect costs.
- Direct costs: (Medical costs) Insurance premiums, patient co-payments, and non-reimbursed medical expenses incurred from hospitalization and pharmacy use by health insurance subscribers and medical aid recipients (Transportation costs) Average round-trip transportation costs for hospitalization and outpatient visits (Caregiving costs) Average caregiving costs incurred when using caregiving services
- Indirect costs: (Loss of productivity due to premature death) Calculated using the number of deaths by gender/disease/age from the raw data on causes of death provided by Statistics Korea, along with survival rates by gender and age, and average wages (Loss of productivity due to medical use) Calculated using the time lost due to hospitalization and outpatient visits and the value of time by age
- (Step 2) Calculate costs for each item

- (Step 3) Estimate the total socioeconomic cost.
- The COI [Cost of Illness] approach encompasses all resources consumed and opportunity costs due to the disease.
- Sum of direct costs directly related to the use of medical services
- Indirect costs from the loss of productivity due to medical service use and death
