Implementation System

- Eligible for registration: Patients with hypertension or diabetes (aged 65 and older, or aged 30 and older who wish to participate in the registration management program).
- Designated Medical Institutions (Initial Registration): Completion of consent forms and registration in the electronic system.
- Designated Medical Institutions (Daily): Medical consultation, reduction of patient co-payment for treatment costs, entry of medical information, and input of the next appointment.
- Designated Pharmacies (Daily): Reduction of patient co-payment for medication costs, medication counseling, and entry of dispensing information.
- Public Health Centers: Management and monitoring of medical and medication expenses.
- Registration & Education Center (Call Center): Program guidance (once), education guidance and reservations, and telephone counseling services.
- Registration & Education Center (In-person Visits): Provision of educational materials, periodic assessment of risk factors, on-site education, and hands-on, experience-based training.
- Information System: The Hypertension and Diabetes Registration Management Information System provides automated support such as medical appointment reminders and notifications for missed schedules.
Meaning: This illustrates a patient-centered system in which medical institutions, pharmacies, public health centers, and education centers are organically connected through a registration management system to provide integrated support, including reduced medical costs, education, and counseling.
Purpose
To suppress and delay the onset of cardiovascular and cerebrovascular diseases by improving continuous treatment and appropriate management for patients with hypertension and diabetes.
Target Population
Hypertension and diabetes patients aged 30 or older residing in the project areas according to their resident registration

- (Medical expense support) Reduction of monthly medical expenses (KRW 1,500/person) and medication costs (KRW 2,000/disease) for patients aged 65 and older Support age and amount may vary by region.
- (Education and counseling) The Hypertension and Diabetes Registration and Education Center provides education and counseling on disease management (theory and practice), nutrition, and exercise programs to improve self-management skills
- (Recall/Reminder) SMSNotification service for upcoming and missed appointments via SMS/ACS
